1. Completed and signed National visa application form
2. One recent passport photo 35×45, colored, with white background
3. A valid travel document – passport (containing at least 2 blank pages, issued within the previous 10 years, and valid for at least three months beyond the intended departure from the Schengen Area)
4. Valid health insurance (from the approved list of Indian travel insurance companies), covering the duration of stay, valid in all Schengen states, with minimum medical coverage of EUR 30,000
5. A confirmed air ticket reservation
6. Mediation letter electronically submitted through MIGRIS (https://www.migracija.lt/en) by the Lithuanian scholastic institution (Visa Office must be provided with the mediation application number)
7. Proof of financial means to live in Lithuania and to return:

• Original private bank statement (last 3 months, duly stamped and signed by the bank)

• Indian income tax return acknowledgment (last 2 assessment years)

• In case of sponsorship – Affidavit of Sponsorship from parents/relatives + their proof of financial means
