Poland VFS Checklist

Documents
Passport – Travel document must be valid at least 90 days after the expiration of visa and not older than 10 years; must have at least two consecutive blank pages; damaged travel documents cannot be accepted.
Visa Application
2.1. Visa application form without blanks, signed twice by the applicant (at least 18 years old may lodge and sign personally; signature must match passport).
2.2. Two color photos – passport type, white background 35mm × 45mm, not older than 6 months.Checklist
Applicant’s Covering Letter – Mentioning purpose and duration of travel, list of attached documents, and other useful information.
Documents Confirming the Purpose of the Trip
5.1. Certificate of admission or continuation issued by an educational establishment (as per Regulation of Minister of Science and Higher Education, 23 Sept 2019).
5.2. Proof of payment for studies issued by the institution; if studies are free, a confirmation from the university is mandatory.
5.3. Documents attesting previous education (originals to be shown at counter):A. Higher secondary certificate (legalized/attested/apostilled by MEA/MFA) – required for first year/foundation admission.
B. Higher studies certificates/diplomas (legalized/attested/apostilled by MEA/MFA) – required for second/third cycle admission.
Flight Itinerary (Reservation Only) – Include travel dates.
Travel Medical Insurance (TMI) – One copy of certificate issued by an approved insurance company.
Must be valid in Schengen area for entire stay.
Minimum coverage: €30,000.
Must cover medical treatment, repatriation, and death.
No handwritten certificates accepted.
Details required: Name of insurance company, policy number, validity dates.
Accommodation Proof – Original document confirming availability of accommodation (lease agreement, dormitory decision, etc.), must include monthly fees.
Financial Means (Applicant or Sponsor) – To cover living (776 PLN/month) + return travel (2500 PLN). One or more of:
9.1. Traveler’s cheques.
9.2. Certificate of credit card limit issued by bank.
9.3. Certificate of funds in a Polish bank/co-op savings association (not Indian bank).
9.4. Scholarship grant confirmation.
9.5. Certificate of employment with salary slips.In case of sponsorship: notarized sponsorship letter confirming responsibility for all travel, living, and accommodation expenses.
Minors
10.1. Permission to travel from parent(s)/guardian(s) not present, legalized/attested/apostilled by MEA/MFA (unless sole custody with court order). One parent/guardian must be present at application.
10.2. Copy of ID of parent(s)/guardians with custody/guardianship.
10.3. Birth certificate (or proof of guardianship) legalized/attested/apostilled by MEA/MFA.Additional Documents – Applicants may submit extra documents (e.g., IELTS ≥ 6.5, TOEFL iBT ≥ 79, CAE ≥ 176) as proof of English proficiency.
Residence Proof – Official document proving stay in the current jurisdiction.
Passport Data Pages Copy
13.1. One copy of applicant’s passport data pages (first and last).
13.2. Copies of pages with Polish/Schengen visas (including from previous passports).
